Remote Web-based Support System and Medication Adherence in Renal Patients

Chronic Kidney Disease (CKD) is a progressive condition among renal patients that can eventually lead to kidney failure. This long-term condition was estimated to have a global prevalence of 9.1% in 2017 and this figure is known to be on the rise. Although the disease is generally asymptomatic in the early stage, progression over years can lead to life-limiting conditions and eventually put a burden on healthcare systems due to the need for transplants.

Therefore, helping people to delay the progression of CKD has the potential to lower burden on healthcare costs. Medication for CKD is known to delay of the progression of the disease. However, non-adherence to prescribed CKD medication is widespread, particularly among those who are largely asymptomatic in the early stages. It has been estimated that only 50% to 70% of CKD patients take their medications regularly.

Recent studies by CARE researchers on effective ways of improving adherence to CKD treatment have looked into remote methods of support. This includes services such as CareKnowDo, which is a multichannel support program developed by a team of doctoral-level psychologists, specializing in health behavior at Atlantis Health, in collaboration with the Renal Association. This service provides people living with CKD with tailored web, SMS, email, and nurse phone support based on key self-management behaviours.

Behavioural change interventions that focus on Self-Regulation are known to be most effective in addressing non-adherence to medication. A latest study, including research by CARE’s Professor John Weinman, focused on evaluating the feasibility of implementing CareKnowDo, integrated with Patient View (an electronic health record) in managing adherence issues among renal patients. This study's results displayed feasibility and acceptability. Participants of the study favoured the content of these web-based self-management programmes. This suggests that remote interventions like CareKnowDo and Patient View can make substantial contributions in increasing adherence among CKD patients with the aim of lowering healthcare burden in the long run.
